Service Description
Receive expert support to: - Understand complex diagnoses or care plans - Review test results, medications, and second opinions - Prepare for provider appointments and provide them with a tool for efficient communication to improve health outcomes - Discuss symptom changes, treatment concerns, and your goals for health and wellness Each session includes a personalized Continuity Compass, a customized care and communication document, to help coordinate care with providers and caregivers.
Cancellation Policy
Cancellations made within 24 hours subject to a 50% fee; no-shows charged 100% Booking Policy Summary We provide advocacy and wellness consulting services—not medical management. While sessions are led by a licensed Nurse Practitioner, no diagnosis, treatment, or prescribing is provided. Booking does not establish a patient-provider relationship. -Scheduling & Payment Sessions must be booked in advance. Payment is due at time of booking and session duration will be arranged during the Discovery call. Services are available virtually or in person (Brevard County only). You will receive an email to complete new client paperwork after your Discovery call. Paperwork MUST be completed prior to your session, or your visit will be rescheduled. If it is determined during the Discovery call that outside medical records are needed, we will be unable to complete your session until those records are received. This will ensure the best use of your time and our services.
