Primary Care Membership
Telehealth-First Care
Ongoing primary care for Florida members, with limited office visits when clinically appropriate.
Whole-Person Support
Care that considers your medical, emotional, and practical needs.
A Separate Care Option
No prior transitional care is required. Membership is a separate option for ongoing primary care.
Connection Beyond the Visit
Health education and care navigation support. Counseling and one-on-one advocacy are billed separately.
