Short-Term Primary Care
Medical care for patients who need support during gaps or transitions in care.

Available by telehealth in Florida and Georgia, in-office in Palm Bay, FL, and through limited home visits for homebound patients in Brevard County, FL
Follow-up after an ER, urgent care, hospital, or facility discharge
