Tiffany James
MSN, APRN, FNP-C, BCPA
Founder & CEO | Family Nurse Practitioner
Tiffany James is a board-certified Family Nurse Practitioner and Board Certified Patient Advocate with more than nearly 20 years of nursing experience and over a decade of advanced practice experience.
Her clinical background includes family medicine, urgent care, community health, rehabilitation, skilled nursing and long-term care, telehealth, and transitional care. This broad experience has given her a unique perspective on what happens when patients move between settings, providers, and stages of care.
Tiffany founded Divine Wellness Partners to help close those gaps. Her approach combines short-term primary care with health literacy, patient advocacy, care navigation, and connection to long-term resources so patients and caregivers feel supported during healthcare and life transitions.
In addition to her clinical work, Tiffany is actively involved in community health, advocacy, and education throughout Brevard County and beyond. She is passionate about improving access to care, strengthening health literacy, and helping patients become more confident participants in their healthcare.
Her goal is simple: to make the in-between moments of healthcare feel less fragmented, less confusing, and more supported.
