Includes 60-Minute Session + Custom Health and Wellness Binder This stand-alone, comprehensive binder helps you organize all essential health information in one place-ideal for seniors, caregivers, or clients desiring an efficient communication tool to keep on hand while navigating healthcare complexities Includes: - Diagnoses, medications, and supplement list - Past medical, surgical, and social history - Insurance information, Important medical documents - Active providers and care teams (with contact info & care purpose) - Next of kin and POA contact information - Emergency preparedness section: designed for first responders and caregivers How It Works: - After Discovery call, submit your information via our secure portal at least 7 days before your scheduled session - Meet for a 60-minute review session - Receive your digital binder customized to your care (Hard-copy binder available for pick up (Brevard County Residents only) for additional $25
Cancellation Policy
Discovery Calls are offered virtually. If you’re unable to attend, please provide at least 48 hours’ notice to reschedule. Specific terms for cancellation are outlined in paperwork each client receives via email. A Discovery Call is required before booking additional services to ensure appropriate alignment and support. All advocacy services with Divine Wellness Partners begin with a Discovery Call. Step 1: Use the booking link on our main page to schedule your Discovery Call at a time that works best for you. Step 2: Once your booking is confirmed, you will receive an email at the address you provided. This email will include important details on how to prepare and next steps. Step 3: After your Discovery Call, we will determine the best service package or support plan tailored to your needs. Please note: A Discovery Call is required before scheduling any advocacy visit or service.
