Medical advocacy means having a trusted partner who understands the healthcare system and speaks on your behalf. We turn medical jargon and rushed visits into clarity. Care navigation ensures you move safely through the system—coordinating providers, preparing for transitions, and making sure your voice is heard.

Our Mission:

At Divine Wellness Partners, we make healthcare easier to understand and navigate. Led by a Nurse Practitioner and Board-Certified Patient Advocates, we provide medical and mental health advocacy and care navigation for patients and caregivers in both inpatient and outpatient settings.

We bridge care gaps, support continuity, and empower health literacy—helping you become a strong self-advocate.
